Over the last 3 years I have had extensive experience of care of the elderly in the Island in the form of my parents. The communication between discharging them from the hospital and adult social care is woeful at best and downright dangerous at its worse. I have meet with Adult Social Care teams,made suggestions and while they have great ideas, they are being prevented from actioning them somehow. My experience was pretty abysmal 18 months ago. Over the last week I have experience again I regret to say, it has worsen. Furthermore, and for perhaps another topic, there is something serious wrong with the communication channels at the hospital which again makes them dangerous and they are putting lives at risk, probably through no fault of the poor staff there who are trying their best.
